Qualifying Process
1. See if you qualify
The first step in applying for your Medical Marihuana Identification Card is seeing if your condition qualifies. The list on the right shows the medical conditions and ailments currently recognized by the State Of Michigan for qualifying for this card.
2. Call us to enroll
If your condition qualifies or you have extenuating circumstances that make you or your doctor feel that you qualify, we encourage you to call +1.517.787.1206 and enroll as one of our patients.
3. Obtain copies of your medical records
After you have enrolled with our clinic and added your name to our list then the next step is for you to get your medical records. You cannot see one of our physicians without current medical records. You can either bring them with you the day of your visit, or you can have them faxed to 517-292-2487.
4. See our physician
Once you have your medical records, or we have received them, we will schedule an appointment for you to see one of our available physicians here at our office for discussing your qualifying condition(s). We will schedule certain days for physician visits. Please note that this visit with the physician is strictly for discussing the qualifying condition for completing the states forms for obtaining the Medical Marijuana Identification Card.
On the day of your scheduled visit, please have a valid Michigan Photo ID, and the $200 fee for the visit.
We do not accept checks.
5. Apply for the Medical Marijuana Identification Card with the State of Michigan
If our doctor agrees that you qualify for medical marijuana under Michigan State Law, you will receive a doctor signed State Certification Form. This form is one of the key documents you need to successfully obtain your State of Michigan Medical Marijuana ID card.
Next, download one of the application packets listed below. Be sure to fill out all forms included in the packet, and include copies of all extra documentation needed. We strongly recommend that you double-check that you are including all of the required documentation and have the forms filled out correctly before sending them to the State of Michigan.

Qualifying Ailments
The specific debilitating medical conditions that qualify a patient to use marihuana for medical purposes are:
• Cancer
• Glaucoma
• HIV or AIDS Positive
• Hepatitis C
• Amyotrophic Lateral Sclerosis
• Crohn’s Disease
• Agitation of Alzheimer’s Disease
• Nail Patella
• Cachexia or Wasting Syndrome
• Severe and Chronic Pain
• Severe Nausea
• Seizures (Including but not limited to those characteristic of Epilepsy.)
• Severe Persistent Muscle Spasms (Including but not limited to those characteristic of Multiple Sclerosis.)
